On November 28th, doctor Cavalieri from INT partner, will present BDQoL in Milan at the event Diagnostic Therapeutic Care Pathways - The pathway of the cancer patient hosted by Regione Lombardia.
The need for integration between hospital and territorial services is an issue that is still but remained substantially unresolved; and this despite the fact that it is increasingly evident how the epidemiological transition, with the increased prevalence of chronic diseases, which accompany individuals for a long time, and the progressive aging of the population, now makes continuity in/of care indispensable for taking care of patients who inevitably require long-term care at different treatment intensities.
In order to meet the challenges posed by the new, increasingly chronic oncology, healthcare must profoundly reshape its operating model by adapting an incremental type of rationality; an adaptive, management-oriented rationality of care processes and care pathways: of a horizontal type for integration between hospital and territory and of a vertical type for functional linkage between facilities with different intensity of care and specialties.
The Diagnostic Therapeutic Care Pathways and its indicators today are the appropriate tools for tracking and measuring the cancer patient's journey between hospital and territory; a model in which all health professionals become an integral part of the national health care system, fostering a continuous osmosis between planning, knowledge acquisition, and multidisciplinarity between the expert center of reference i.e., the hospital and the various professionals involved in the territory in a true delocalization of care and taking charge.
